1. File a claim with Berkshire Hathaway - Use the information provided in the attached flyer to file your workers' compensation claim directly with Berkshire Hathaway. 
2. Complete an Incident Report for Shearwater Records - Submit an internal incident report by scanning the QR code or using the Formstack link included in the flyer. This ensures the incident is properly documented within Shearwater's records. 

Timely and accurate reporting helps ensure your claim is processed efficiently and your records are complete.
